Many local health departments, hospitals, and their community partners struggle with the implementation of their Community Health Improvement Plan (CHIP)/Implementation Strategy (IS) after it is developed. This is unfortunate because the implementation of the CHIP is the reason the Community Health (Needs) Assessment (CHA/CHNA) and CHIP/IS are done in the first place! Challenges with CHIP/IS implementation include translating CHIP/IS strategies into tangible outcomes (often due to rushed or limited planning processes), lack of staff, not knowing what to do next, staff turnover, lack of funding, and many more reasons. These challenges are NOT because communities do not want to achieve health improvement!
